Community Health Needs Assessment (CHNA) & Implementation Strategy (IS)


About the FY2019 CHNA and FY2020-22 IS

BID Plymouth's most recent Community Health Needs Assessment was conducted in 2019. Findings from the 2019 CHNA indicated that the hospital should prioritize populations with complex needs and/or significant barriers to care, service gaps and adverse social determinants of health.

BID Plymouth's Community Benefits program is currently focused on the following service areas, populations and health priorities:

Target Population

  • Youth and families
  • Older adults
  • Low- to moderate-income individuals
  • Individuals with chronic/complex conditions

Prioritized Health Needs

  • Chronic/complex conditions and their risk factors
  • Behavioral Health (mental health/substance use)
  • Access to Care
  • Social Determinants of Health (food insecurity, housing stability)
