Two Options: Tufts Health Plan HMO, Harvard Pilgrim Health Care HMO; both plans require 15 or more hours for eligibility and take effect 30 days after date of hire. Open enrollment date is during the month of September (specific date announced each year) for an October 1 effective date.
Buy back: When certain eligibility rules are met, staff may collect a bonus for dropping health insurance for at least one year.
Staff in SEIU-represented positions and managerial staff are eligible to select from two Delta Dental Plans:
Level I: Preventive maintenance plan-$300 yearly coverage
Level II: 100% preventative, 80% basic restorative, 50% major restorative-$1000 yearly coverage.
Staff in MNA-represented positions are eligible for Blue Cross/Blue Shield Dental Blue - 100% preventative, 80% restorative, 50% major restorative- $1000 yearly coverage.

Time off with pay for scheduled staff; three or five days off with pay depending on the relationship to the person who passed on, consistent with union contracts and hospital policy.
Eligibility requirements for health and dental plans have been expanded by the hospital to include domestic partners.
Flexible Spending Account:
Set aside untaxed money to fund dependent care or health/dental expenses. Open enrollment is during the month of December for a January 1 effective date.
Eligibility - scheduled position of 30 or more hours/week for 6 months. Benefit begins 90 days after date of injury and equals 60% of base monthly income.
Free Group Term Life Insurance:
Life Insurance: For staff holding position of 20 or more hours per week. Accidental death or dismemberment - double indemnity. HIV INSURANCE: All RN's covered. Supplemental coverage may be elected and paid through payroll deduction.
Staff with scheduled hours can be reimbursed for successful completion of courses approved by their manager. Yearly limits apply.
Staff with scheduled hours who are enrolled in Registered Nurse nursing programs qualify. This program is funded by the Beth Israel Deaconess - Plymouth Club.
An hourly differential is paid for weekend, evening and night work.
Contributory 403(b) plan, immediate vesting; hospital contributions after 1 year with eligibility requirements met, 1000 hours paid in four consecutive quarters. Plan administered by Fidelity Investment (800-343-0860) for changes in contributions you make, changes in investment selection and loans.
Individual Tax Sheltered Annuities:
The hospital will accommodate supplemental deductions to individual plans.
U.S. Savings Bonds:
Series 1 and Series EE bonds can be purchased through payroll deduction. Sign up in Human Resources Department.
Paychecks can be electronically deposited to any personal account. (Multiple deposits are accommodated).
Supplemental Life Insurance:
Term insurance is available through payroll deduction. Spousal and child coverage is also available.
Prorated for 15 or more hours.
- 22 days annually under 6 years of service
- 27 days annually after 6 completed years of service
- 32 days annually after 24 completed years of service
Eligibility for use after 90 days. Once eligible, upon resignation, earned time will be reimbursed.
Earned Time Buyback:
This is offered twice a calendar year, up to 64 hours (total) may be cashed out in lieu of time off.
Holidays are prorated for 15 or more hours; 10 legal holidays plus employee's birthday. (New Years Day, Presidents Day, Patriot's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day, Christmas Day). Legal holidays worked are paid at 1 1/2 rate.
On-Site Cafeteria for Those Working on Campus:
The food service is subsidized by the hospital and includes take-out service.
Plymouth Fitness & Jordan Health & Wellness: subsidized enrollment fee and reduced monthly fee paid if paid via payroll deduction.

Employee Assistance Program:

$100 per year for full time staff with 10 years of completed service; $200 for 25 years of completed service (pro-rated for part-time schedules of 15 or more scheduled hours).

With hospital approval, staff may increase or reduce weekly scheduled hours through a "jobshare" with fellow scheduled staff members or with the hospital; up to 8 hours per week, 3-month minimum. Benefits adjusted accordingly.

Yearly recognition for 2 staff members nominated by fellow staff, who demonstrate positive qualities as defined by the former CEO's example. $250 award and crystal clock.
