Reporting a Privacy or Data Issue or Concern
The Beth Israel Deaconess Hospital-Plymouth values the privacy and security of your personal and protected health information. If you believe that your information has been compromised in any way, please let us know immediately by choosing one of the available options below.
How to Report a Privacy or Data Concern
You may report a privacy, confidentiality, or data concern in a variety of methods:
a. Contact a supervisor, manager, or director.
b. Telephone a member of the Office of Compliance and Business Conduct at (508) 210-5942 or 5943.
c. E-mail our Privacy Officer at: firstname.lastname@example.org.
d. Telephone the Privacy Helpline at (888) 753-6533 which is administered through a helpline service. Callers have the option of identifying themselves or remaining anonymous. Helpline users will be provided feedback by Helpline staff who will relay the response on behalf of BID-Plymouth within the specified timeframe quoted by the helpline service.
e. Complete a Privacy Helpline form electronically at https://bidmccompliance.alertline.com
f. Send a fax to the Office of Compliance and Business Conduct at (508) 210-5367.
g. Complete a Privacy and Security Form.
h. Call the IT Support desk at (508) 830-2038
All callers will not face retaliation or retribution for reporting a concern. This is the law.
We will strive to maintain the confidentiality of a caller’s identity, however, there may be a point where an individual’s identity may become known or may have to be revealed.
